Failure mode and effects analysis (FMEA) is a system used in order to evaluate processes and determine the possibility of failure, as well as what impact such failures would have. An FMEA process includes close examination of processes, failure modes, the causes of failures and their possible consequences. An FMEA report is aimed at averting failures and reducing accompanying risks. FMEAs are particularly important when new manufacturing techniques and processes are introduced. It should be closely linked to and affect the design process in order to correct possible failure modes as cost effectively and as thoroughly as possible.

An FMEA is conducted by a cross-functional team consisting of individuals who specialize in various manufacturing and supply chain aspects of a process. Flowcharts are generated to outline the scope of the study, and FMEA information is recorded on a spreadsheet. Team members strive to identify every possible potential failure in their areas of specialty, as well as the potential causes and effects of such failures. Process controls are designed to eliminate the causes of potential failures. Controls are allocated a detection rating that indicates how effectively the control would identify possible deviations. A risk priority number or RPN is calculated for each potential failure and recommendations aimed at minimizing failures are made.
